- How do I submit a name change to HR?

- I can't log into EKU Direct. I need to have my EKU Direct pin reset.
- What is the process for employee discipline?
A copy of your social security card reflecting the name change must to be submitted to HR.
Log into EKU Direct or use the Address Change form. Make the needed corrections on the change form and mail to the HR Office. Forms are also available at HR.
Retirement system members should also complete the KERS or KTRS address change form and forward to the HR retirement specialist.
Submit an updated Federal W-4 or State K-4 and mail to the Payroll Office (Coates CPO 3A. We suggest contacting your tax advisor if you have questions about claiming exemptions.
Direct deposit is required for all EKU employees. Submit the completed Direct Deposit form to HR Office. A voided check or documentation from the bank indicating the account and routing number must be attached to the form. Once the direct deposit is set up, it must pre-note (a practice run) for one pay period and should be direct deposited by the next pay date. Because of this, please verify that your pay has been posted to your account prior to spending.
Request for verification can be faxed to Sheila King at 859-622-6667 or delivered to the HR Office. A contact number is required for notification that the verification is ready.

The position action form (PAF).
The individual must apply using the on-line employment system. Once the process is complete, the person must complete a W4, K4, I9 and self-identification form.

It is a federal requirement for employment in the United States. All new EKU employees and employees returning to work after a one-year separation must complete the Federal I-9 form. Section 1 must be completed by the employee on the first day of employment. The list of required documents supporting eligibility to work is on page 3 of the form; original copies of the supporting documents must be submitted to HR within 3 days of employment.
Yes. Payroll requires a copy of an employee´s Social Security Card to help ensure W-2 information is reported correctly. If the I-9 requirements are met with other documents and the card is not available, a receipt for application from the local Social Security office can be accepted initially. Once the new card is received, bring it to the HR office so a copy can be placed in our records.
New benefited staff and faculty employees attend New Employee Orientation. New faculty also participate in New Faculty Orientation. An orientation date is scheduled by the HR Employment Analyst upon completion of the hiring process and a welcome letter is sent to the employee with orientation details. Upon notice that an employee intends to separate from employment, the employee´s department must notify HR via a PAF reflecting separation. The department should also submit a completed Separation Notice to HR. For more information see Employee Separation.

Completed forms should be delivered to Glenna Flannery's attention at HR. She will verify eligibility, make a copy for the personnel files and forward the form to the scholarship office. HR is able to fax forms to some institutions, but we recommend that you deliver them to the institution

Changes can be made during open enrollment or within 30 days of a qualifying event. Contact the Benefits staff for explanations of a qualifying event. Changes should be mailed to the Benefits Office, Coates 24A.
The employee is responsible for the deductible plus 5% on Inpatient Services, Outpatient Services, Mental Health Services, Home Care Services, and Maternity Services. For example, if the employee has a surgical procedure done, he/she would pay the $250.00 deductible. He/she would also be responsible for an additional 5% of the cost of the procedure up to a total of $1,250 maximum out of pocket. This 5% is called a coinsurance. Contact the Benefits staff for further explanation.
Contact Tarena Tyree to request the appropriate form. If approved by the Sick Bank Committee, you will be notified by the Benefits Office and they will enter your time into the system immediately. You should continue to submit your time card indicating the appropriate sick hours.

The employee wellness program is designed to improve employee health through education, intervention and encouragement of healthy lifestyles. It provides a range of opportunities to faculty and staff from educational seminars and wellness challenges, to health fairs and preventative screening opportunities. Your participation the any Healthy You! At EKU wellness activity is completely voluntary and confidential and we are committed to protecting your privacy. All personal health information that you record on the Healthy You At EKU web site is stored on the secure servers of Matria WellCare and will not be available to anyone at your organization. - How do I get started in the wellness program?
Log on to HealthyYouatEKU (using your employee ID number) and create a profile. Get a jumpstart on earning credits by completing the online Wellness Assessment worth 5 credits. Watch for campus notices and log in frequently to see upcoming activities and events. - What can be counted as a Campus/Community Event?
A campus/community event is any event that involves physical activity such as a not-for-profit walk or run, Habitat for Humanity, Special Olympic volunteer or other eligible community or campus-sponsored event. - How do I log my wellness points?
All points (on-campus seminars, health fair participation, campus/community events, quarterly challenges, etc.) for the wellness program are recorded and tracked on our website. To enter your points, log onto HealthyYouAtEKU under the Add and Enter Credits link. Your points must be entered via the web site by the end of the program year (February 28, 2009) to earn your reward. - How do I know what the upcoming activities are?
All upcoming activities are listed on our website, HealthyYouAtEKU. On the home page you will find a Healthy You! At EKU Bulletin Board. Here you will find a link to Credit Opportunities Available Now. Check back frequently to see upcoming activities and events. - I´m having problems creating my profile, what should I do?

The Staff Development Fund provides matching funding for eligible staff to participate in professional development events/activities that support individual and University development. For more information visit Staff Development Fund.
- Who has to have a performance evaluation completed?
- When are performance evaluations due?
We provide a variety of development opportunities for effective work performance, quality of worklife, and personal and professional growth. HR also provides customized training and development upon request and based upon University needs. For more information visit HR Employee Development
All full- and part-time staff employees must be evaluated under the University’s Employee Evaluation system. Performance Planning is completed with staff employees upon successful completion of the 90-day orientation period. All eligible staff are evaluated in January for the previous calendar year. For more information visit Performance Management.
Annual Performance Reviews for the 2007 calendar year and Performance Planning for 2008 (for staff) must be completed not later than January 31, 2008. The original completed 2007 Performance Evaluation form is due to HR for filing not later than March 1, 2008.
